4) Please provide your date and time of birth. If you are a twin, please let me know, and if you are born overseas, please provide your birth country.
5) Please note that payment will not be refunded if cancellation is made without a replacement. Please also note that if you miss the appointment and wish to have another slot, this new slot will be charged at the same price as above.
6) Each slot is about 1.5–2 hours, depending on the individual.
